Then print it, sign it, and mail it in !!

Thank you for taking the time to fill out this application. This is the last step, where we create a Printable Copy of your Application. To do this, Adobe Acrobat Reader must be installed correctly on your computer. If you do not have this program, you can download it here.

You must sign your application and mail it too us so that we can process your credit card payments to your bank account. Information on where to sign and send the application is available here and on the instruction sheet. If you should should require any assistance, please do not hesitate to contact us at the number below. 

After clicking on the continue button, please be patient for the application to generate and appear on your screen. Once the form is displayed, print it (see Notice for New Acrobat Users), sign it as indicated, and click on any Main Menu selection to close the application window. Please note that all the information to process your merchant account is already printed on the application. You do not need to add additional information.

You will also receive this PDF file by e-mail at the address you gave to us in this application.
